The aim of this study was to analyse the efficiency of intratympanic methylprednisolon injection (ITMI) to treat sudden sennorinaural hearing loss (SSNHL) as a salvage treatment in patients who failed initial systemic steroid treatment. 57 patients with SSNHL who did not respond to initial systemic steroid were prospectively included in the study. The patients were randomly classified into two groups, the ITMI group (28 patients) was performed twice a week for two consecutive weeks, and the control group (29 patients) took any more medications. A 10 dB HL improvement in the PTA was defined a successful treatment. Hearing improvement was observed in 36,7% of ITMI group and 18,8% of control group. An improvement of the mean PTA was 9,7 dB in the TIMI and 4,1 dB in the control group. The ITMI group showed significant hearing improvement at low frequency, especially at conversation frequency (1,000 Hz) than the control group (p = 0.02). The sequential ITMI which is performed immediately after initial systemic corticoid therapy, may be an effective second-line treatment of choice for patients who show poor response to initial treatments for SSNHL.
